4. Documents needed
   • One month most recent pay stubs or two most recent pay stubs, if paid monthly
   • Names/addresses of employers for two years
   • W-2s/1099’s for two most recent years
   • Two most recent years of tax returns (all pages Federal Only)
   • Most recent 401K or TSP quarterly statement (all pages)
   • Bank statements for two months, all pages
   • If self-employed, year-to-date profit and loss statement with balance sheet, plus signed returns for last two years(all schedules)
   • Proof of pension income, if applicable
   • Social Security and Disability payments, if applicable
   • Dividend earnings
   • Copy of most recent mortgage statement, Tax bill, Insurance declaration page, HOA Statement, and lease agreement(If applicable)
   • Copy of Divorce decree showing any child support and/or alimony payments (If Applicable)
   • Legible copy of driver’s license, social security card, and resident alien card (If applicable)
